Mobility documents for academic year 2024/2025  

Please, send all these documents via email to international Exchange Office to be corrected before you send them to the receiving institution. 

• Learning Agreement for Traineeship, LAT – including the programme of your traineeship 

• Insurance coverage and European Health Insurance Card (EHIC)– your insurance coverage should be valid for the whole mobility period including travel days. 

 NOTE!

Your insurance coverage should include at minimum:  

• Health insurance (the participant undertakes to obtain health insurance coverage valid in the country of traineeship; in another EU country (to obtain European Health Insurance Card), in non-EU countries to purchase a private insurance corresponding in scope to European Health Insurance).  

• Accident insurance (covering at least damages caused to the student at the place of traineeship/studies) 

• Liability insurance (covering damages caused by the participant at the place of traineeship/studies)  

The coverage of the European Health Insurance Card or private insurance may not be sufficient, especially in case of specific medical intervention or transport to the country, therefore a complementary private insurance is recommended. In addition to the above, insurance against loss or theft of documents, travel tickets or luggage is recommended.  

A mobility participant is responsible for purchasing his/her own insurance policy and submitting it in International Exchange Office not later than one week before the start of mobility period or sent by electronic message (a good-quality scan copy). The copy of the insurance coverage purchased by the participant is an integral part of this Agreement. 

• OLS test result 

• Green travel application – submitted ONLY when travel is planned using low-emission means of transport such as a bus, a train or carpooling) 

• Letter of intent – if the traineeship is planned in the institution other than University  

MOBILITY DOCUMENTS –  TO BE DOWNLOADED:  

Extension of your mobility period is possible upon the consent of both universities.  

NOTE! 

• Your mobility period cannot last longer than 12 MONTHS 

• an additional scholarship for this period is not guaranteed and it depends on the fianancial allocation
• You can apply for this opportunity but NOT LATER than one month before the ending date of your mobility (as previously agreed in your GRANT AGREEMENT). 

HOW TO APPLY? 

1. Contact the person responsible for your mobility at the receiving institution, your Faculty Coordinator and Erasmus office at University of Life Sciences in Lublin for their acceptance 
2. Make sure that all the extension documents are signed by all parties at least one month before the previously agreed dates of your mobility
3. Extension of your study period without the permission of all parties will not be accepted. 

THE DOCUMENTS REQUIRED IN CASE OF THE EXTENSION OF STUDY PERIOD: 

It is your responsibility to obtain the required AFTER MOBILITY DOCUMENTS from the receiving institution to submit them in International Exchange Office not later than 2 weeks after the end of your mobility: 

• TRAINEESHIP CERTIFICATE  

• CONFIRMATION OF STAY AND TRANSCRIPT OF WORK (if the confirmed period of time is different from the one indicated in your Grant Agreement, the financial conditions of your mobility may change accordingly, the Transcript of work should include grades appointed to you by your mentor)   

• EU-SURVEY – to be submitted online after your mobility  

• POWERPOINT PRESENTATION with high quality photos (if possible) 

• A STUDENT STORY IN A WORD FILE– to be published on ULSL Erasmus website and ULSL journal “Aktualności”.  

• GREEN TRAVEL -AFTER MOBILITY –ONLY if applicable 

1. Extension of study period letter– should be addressed to Erasmus + Institutional Coordinator, provide the justification for extension and new exact dates of your mobility period Possible additional funding will be calculated based on the new dates provided. Before submitting the document at International Exchange Office, make sure to get approval from your Faculty Coordinator 
2.  Application for extension of traineeship period signed by all parties  

ALL THE DOCUMENTS NEED TO BE SUBMITTED AT INTERNATIONAL  EXCHANGE OFFICE NOT LATER THAN ONE MONTH BEFORE THE PREVIOUSLY AGREED DATES OF YOUR MOBILITY. 

After the documents are approved by the Faculty Coordinator and the University Coordinator, you will be sent an annex to your grant agreement to be signed.   

EXTENSION OF YOUR MOBILITY WITHOUT AN APPROVAL FROM BOTH SENDING AND RECEIVING INSTITUIONS WILL NOT BE ACCEPTED.

DIGITAL SKILLS TRAINEESHIPS: 

Students and recent graduates (regardless of your field of study) have the opportunity to undergo digital skills traineeships. 

The programme of digital skills traineeship must involve one or more of the following activities: digital marketing (e.g. social media management, web analytics); computer graphics, mechanical or architectural design; creating applications, software, scripts or websites; installation, maintenance and management of IT systems and networks; cybersecurity; data analysis, visualization; robot programming and AI applications.  

This category does not include traditional customer support, customer orders management, entering data or regular office tasks.
