• Transcript of academic records provided by your university (list of courses, exams and grades from your home university)
• Certificate proving sufficient knowledge of English (minimum B1 or adequate level)
• A photocopy of the applicant’s ID or passport
• Accommodation form (if you wish to stay in our student dormitory)

IMPORTANT NOTICE!

The forms must be typed! Handwritten forms will not be accepted. 

A complete set of all these documents must be sent without signatures first in Microsoft Word format in order to be verified.

Once you have been accepted by the receiving institution, remember to sign the forms and get them signed by your university! You will receive an official Letter of Acceptance along with all the necessary information.

LANGUAGE REQUIREMENTS:

Be aware of the fact that your level of English must be good – at least B1. It means that you must be able to understand, speak and write in English. You will not succeed in most subjects if your level of English is poor.

ADDITIONAL DOCUMENTS REQUIRED FROM STUDENTS ACCEPTED BY ULSL:

• a Grant Agreement – signed, should be emailed to .  A draft of the agreement is prepared by ULSL International Exchange Office (IEO) and sent via email to the Partner University and the student.
• A scanned copy of students’ insurance coverage for the period of travel and stay at University of Life Sciences in Lublin. Your insurance coverage (English version) should include at minimum health insurance, liability insurance (to cover damages caused by the participant at the place of mobility), accident insurance (covering at least damages caused to the student at the place of mobility). The responsible party for taking the insurance coverage is the mobility participant. The confirmation of insurance coverage (in English) purchased by the participant shall be emailed to the sending organisation (Erasmus Office) not later than one week before the start of mobility period (a good-quality scan copy). The copy of the insurance coverage purchased by the participant is an integral part of this Agreement and the condition to have your scholarship paid out.

• Green Travel Application  APPLICABLE ONLY TO GREEN TRAVEL OPTION
• 1 passport-size photograph and a good-quality digital photo

To complete your mobility and have it fully recognised at your sending University you will have to take the following steps:

1. Make sure you receive Transcript of Records (mobility for studies) or Traineeship Certificate (mobility for traineeship) from ULSL (issued to you on the condition that you have successfully completed your mobility programme.
2. Fill in PARTICIPANT REPORT (EU Survey)– the link to the survey will be sent to your email address by the  end of your mobility.
3. ONLY For Green Travel Option – fill in AFTER MOBILITY BEFORE DEPARTURE Green Travel statement
