INFORMATION CLAUSE

Pursuant to Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation – GDPR), and the Act of 10 May 2018 on the protection of personal data (consolidated text: Journal of Laws of 2019, item 1781), the University of Life Sciences in Lublin informs that:

  1. The controller of your personal data is the University of Life Sciences in Lublin, ul. Akademicka 13, 20-950 Lublin, Poland.
  2. The Personal Data Controller has appointed a Data Protection Officer, who supervises the correctness of personal data processing and can be contacted via e-mail at: buchlinska@up.edu.pl.
  3. Your personal data will be processed:
    1. for the purposes of recruitment and participation in the PROM Project. The legal basis for processing personal data is Article 6(1)(a) GDPR – consent to the processing of personal data. Consent may be withdrawn at any time by sending an e-mail to: prom@up.edu.pl;
    2. in order to fulfil the obligations incumbent on the Controller in connection with the implementation of the recruitment process and participation in the PROM Project, including documentation of this process, performance of reporting obligations (e.g. to NAWA), archival, statistical, accounting and financial obligations, carried out, inter alia, on the basis of the Act on Higher Education and Science and executive acts issued thereunder, the Act on the National Archival Resources and Archives, and the Act on Public Statistics – pursuant to Article 6(1)(c) GDPR.
  4. Recipients of your personal data include the University of Life Sciences in Lublin, including employees and persons authorised by the Controller to process personal data within the scope of their official duties.
  5. Personal data may also be disclosed to the Polish National Agency for Academic Exchange (NAWA), as well as to entities commissioned by the University of Life Sciences in Lublin to perform tasks under the Project, including third parties providing services necessary to achieve the purposes for which personal data are processed (e.g. IT infrastructure maintenance, technical support, legal services), as well as to entities authorised under applicable law.
  6. Your personal data will be stored for the period necessary to achieve the purposes referred to in point 3, and thereafter will be archived.
  7. You have the right to access the content of your data and, subject to the provisions of law, the right to rectification, erasure, restriction of processing, data portability, the right to object, and the right to withdraw consent at any time.
  8. You have the right to lodge a complaint with the President of the Personal Data Protection Office if you believe that the processing of your personal data by the University violates data protection regulations.
  9. Your personal data will not be processed in an automated manner and will not be subject to profiling.
  10. Providing personal data is voluntary; however, failure to provide such data will make it impossible to participate in the PROM Project.
